Where do strokes come from?

The thromboembolic strokes we discuss cause around 88% of all strokes.)

Over the last 10 years, the aorta has been recognized as another important source of stroke. The aorta is the main artery of the body whose branches go to the head, arms, and legs.

Atherosclerotic plaque is a live tissue that, through poor diet, inactivity, high cholesterol, overweight, etc., grows and becomes progressively more unstable. At some point, plaque fragments. Little bits break away, traveling to the brain. Fractured plaque also exposes its deeper structures to flowing blood, triggering blood clot formation, which in turn can also fragment and go to the brain. Atherosclerotic plaque is a prerequisite for the most common causes of stroke.

If the majority of strokes originate from plaque, why not measure plaque to determine if you're at risk for stroke? How can we easily, safely, and accurately measure plaque in the carotid arteries and aorta? And if plaque can be measured, can it be shrunk or inactivated to reduce or eliminate risk for stroke?

How can plaque be measured?

Just 20 years ago, the only practical method of identifying plaque in the carotids or aorta was through angiography, requiring catheters inserted into the body to inject x-ray dye. Angiography was impractical as a screening measure.

CT scanning and magnetic resonance imaging (MRI) are emerging as exciting methods of imaging both carotids and aorta. Unfortunately, most centers and physicians are much more focused on the diagnostic uses of these technologies for people who have already suffered stroke or other catastrophe, and application of these devices for preventive uses is still evolving. One exception is when aortic calcification or aortic enlargement is incidentally noted on the increasingly popular CT heart scans; this is an important finding that can signal presence of aortic plaque.

The one test that is widely available and can be performed in just about any center is carotid ultrasound. It's simple, painless, and precise. Two basic observations can be made:

1. Plaque detection--Atherosclerotic plaque can be clearly visualized. If plaque blocks more than 70% of the diameter of the vessel, or if there are "soft" (unstable) elements in plaque, then stroke risk may be high enough to justify surgery or stents. However, if there are plaques that are less severe, substantial risk for stroke may still be present that can be reduced with preventive measures.

2. Carotid intimal-medial thickness--This is a measure of the thickness of the lining of the carotid artery in areas not involved by plaque, but often precedes the development of mature plaque. Carotid intimal-medial thickness also provides an index of body-wide potential for atherosclerotic plaque that can place you at risk for stroke. The aorta, for instance, cannot be well imaged by surface ultrasound but can still be a source for stroke. Increased carotid intimal-medial thickness and carotid plaque are closely associated with likelihood of aortic plaque. The Rotterdam Study of 4000 participants demonstrated that if carotid intimal-medial thickness is greater than normal (1.0 mm), then you can be at risk for stroke (and heart attack), even if no carotid plaques are detected.

Carotid ultrasound is the one test you should consider that provides the most information with least effort. Ultrasound is harmless, painless, and can be obtained just about anywhere. Even if your doctor disagrees with your request for a carotid ultrasound, an increasing number of mobile services are popping up nationwide that make this test available for around $100. One important point: many scanners and interpreters will only report whether plaque is present or not. While this is important information, you should request that the carotid-intimal medial thickness be made as well. Not all centers can make this simple measure (because of software requirements), but it doesn't hurt to try. Any amount of carotid plaque is reason to follow a preventive program, even if the plaque is insufficient to justify surgery.

Can plaque be reduced?

Can we shrink plaque in carotid arteries and aorta and thereby reduce, perhaps eliminate, these sources of stroke? That question is gaining momentum as effective therapies become available that pack real punch for reducing plaque.

Study after study has now documented that plaque can be reduced and, with it, risk for stroke. Reduction in plaque of 10-20% is possible within a year or two. Let's consider the most potent influences on carotid and aortic plaque growth that need to be considered in a plaque-reducing program. (I assume that you are a non-smoker--if you are a smoker, you first need to concentrate on quitting.)

Hypertension

More than high cholesterol, various lipoprotein abnormalities carry greater risk for carotid and aortic plaque growth and stroke. Lipoproteins are fat-carrying proteins in blood that cause plaque growth. Patterns which are instigators of plaque growth and stroke include:

o Small LDL particlesSmall LDL particles cause carotid plaque growth more than large LDL particles. This abnormality also triples heart attack risk.

o Intermediate-density lipoproteins (IDL)These triglyceride-rich lipoproteins (present even when triglycerides are low) are a measure of how effectively you clear fat from blood after a meal. IDL is a potent driver of carotid plaque growth. Increased IDL also creates fat-rich plaque that makes it more prone to fragment.

Lower LDL cholesterol may, therefore, be better.

Treatment Strategies to Reduce Carotid and Aortic Plaque

The essential question: How do we reduce carotid and aortic plaque? If we make this the focus of our efforts, many pieces begin to fall into place. If you've had any measure of carotid or aortic plaque such as a carotid ultrasound or aortic calcification on a CT heart scan, you know that you're at increased risk for stroke. You also have a baseline for future comparison to gauge whether your program is working or not.

Because most people have not one but several causes of carotid and aortic plaque, there is no one single treatment that effectively eliminates risk for stroke. Instead, most people require a comprehensive program of healthy diet, exercise, supplements, and medication when indicated. Here, we focus on the nutritional supplements that can be critical components of your plaque-reduction program.

Fish oil

Fish oil is a cornerstone of your stroke prevention program. Epidemiological observations suggest a strong relationship of fish intake and reduction of stroke risk. Carotid ultrasound studies demonstrate less carotid plaque with greater intakes of fish.

A cleverly designed University of Southampton study made the fascinating observation that fish oil transforms the structure of carotid plaque. 150 people with severe carotid plaque scheduled for carotid endarterectomy (surgical removal of the plaque) were given fish oil, sunflower oil, or no treatment over several months while waiting for their procedure. (Delays in the British health system permitted this unique design.) Plaque was removed at surgery and examined. Participants taking fish oil had reduced inflammation in plaque and thicker tissue covering the fatty core, markers of more stable plaque. Those taking sunflower oil or no treatment had unstable plaques with greater inflammation and thinner, less sturdy covering tissue. This suggests that fish oil stabilizes carotid plaque, making it less likely to rupture and fragment.

A standard capsule of fish oil (containing 300 mg of EPA + DHA) contains the same amount of omega-3s as a 3 oz serving of cod or halibut; three capsules (900 mg DHA + EPA) contain the equivalent of a serving of farm-raised salmon. The dose that seems to provide greatest protection from stroke, lowers triglycerides (that form abnormal lipoproteins; see above), and reduces fibrinogen, is four capsules per day (1200 mg EPA + DHA).

Coenzyme Q10 (CoQ10)

Although there are no data specifically addressing whether CoQ10 reduces plaque, it is a marvelously effective way to reduce blood pressure, one of the crucial factors causing carotid and aortic plaque growth. A pooled analysis of eight studies showed that, on average, CoQ10 in daily doses of 50-200 mg reduced systolic blood pressure by 16 mm Hg, diastolic pressure by 10 mm Hg. Data suggest that CoQ10 can reverse abnormal heart muscle thickening (hypertrophy), another manifestation of high blood pressure, strongly suggesting that CoQ10 has benefits beyond just reducing pressure.

Supplements to correct the metabolic syndrome

Weight loss is, without question, the most immediate and direct path to correction of this dangerous pre-diabetic condition. A drop of even 10-20 lbs yields improvements across the board: increased sensitivity to insulin, increased HDL, and reductions in triglycerides, CRP, fibrinogen, small LDL particles, and blood pressure. Diet and exercise are fundamental components of an effort to lose weight; low carbohydrate or reduced glycemic index diets (e.g., South Beach or Mediterranean) rich in fibers are clearly effective. Several supplements can amplify weight-reduction efforts and be useful adjuncts to your lifestyle program. Among them:

This adrenal hormone is key to maintaining physical stamina, mood, muscle mass in men, and libido in women. A recent randomized, placebo-controlled study at Washington University in 56 subjects showed a 13% decline in abdominal fat (fat that drives resistance to insulin) measured by MRI with 50 mg of DHEA per day at bedtime, along with improved sugar control and lower insulin levels.

Pectin, beta-glucan

Pectin is the soluble fiber in citrus rinds, green vegetables, and apples, also available as a supplement. Beta-glucan is the soluble fiber of oats and is also available as a supplement. Both are wonderful fibers that provide feelings of fullness, lower cholesterol, slow release of sugars, and can yield modest weight reduction. A USC study in 573 subjects using carotid ultrasound showed that greater intake of healthy fibers like pectin and beta-glucan is associated with less carotid plaque growth.

Folic acid, vitamins B6 and B12

Dr. Daniel Hackam at the Stroke Prevention and Atherosclerosis Research Centre in Ontario conducted a study using carotid ultrasound in 101 participants treated with folic acid 2.5 mg, vitamin B6 25 mg, and B12 250 mcg per day. Treatment resulted in plaque reduction, especially when homocysteine levels exceeded 14μmol/l at the start, compared to untreated participants who experienced substantial plaque growth.

An attempt to clarify the role of homocysteine treatment was made through a National Institute of Health-sponsored study of stroke prevention. 3680 participants with a prior history of stroke were enrolled and given either a "low-dose" (20 mcg folic acid, 0.2 mg B6, 6 mcg B12) or a "high-dose" (2.5 mg folic acid, 25 mg B6, 400 mcg B12) regimen. Although starting homocysteine levels showed a graded association with stroke risk (higher homocysteine levels predicted greater stroke risk), the treatment groups experienced, on average, only a 2 μmol drop in homocysteine levels and no reduction in stroke risk over two years. The study investigators as well as critics have suggested that the study failed due to an insufficient treatment period and that the doses were too low. (The doses we use in our plaque reduction program are folic acid 2.5-5.0 mg, B6 50-100 mg, B12 1000-2500 mcg.)

L-arginine

L-arginine can be used to overpower the adverse effects of ADMA. L-arginine is emerging as an important carotid plaque-reversing tool. Early reports in animals showed that l-arginine completely halted growth of aortic plaque, and did so more effectively than lovastatin (a cholesterol-lowering drug).

In humans, L-arginine reduces blood pressure, abnormal constriction of carotid and coronary arteries, blocks entry of inflammatory cells into plaque, increases sensitivity to insulin, and heightens exercise capacity. Following coronary angioplasty or stent placement, l-arginine results in up to 36% reduction in plaque growth.

The average American takes in 5400 mg of l-arginine through food every day. Supplementing with doses of 3000-12,000 mg per day has proven useful to correct many of these phenomena. (We use a dose of 6000 mg of l-arginine powder, twice a day on an empty stomach, dissolved in water, for our plaque regression program.) Does this result in a reduction of stroke risk? The emerging data suggest that l-arginine is likely to exert a powerful plaque-reducing and stroke-preventing benefit, but we await more clinical trial data.

Conclusion

Reducing stroke risk by reversing carotid and aortic plaque is becoming an everyday reality, with better tools becoming available. To know whether you're at risk, the best and most available imaging tool is carotid ultrasound, aiming to identify intimal-medial thickness >1.0 mm, or carotid plaque. Any degree of calcification of the aorta, such as on a CT heart scan, is another useful measure of risk.

Treatment to reduce risk is multi-faceted but is based on examining all your sources of risk, including metabolic syndrome, small LDL, lipoprotein(a), and C-reactive protein. Fish oil is the one absolutely crucial ingredient in any stroke prevention program. Other supplements can be used in a targeted fashion, depending on the causes identified for your carotid or aortic plaque. Ideally, repeat scanning of your carotids should be done sometime after your program has begun to assess whether you've successfully achieved reversal of plaque growth.

For the purpose of this report I am concentrating on the maturity-onset type where often due to prolonged obesity, there occurs a reduction in insulin receptors in target cells (i.e., a down-regulation of the receptors), due to high and steady insulin production. Each of us has a specific number of fat cells which is fixed in infancy. As we gain weight, these cells become enlarged and the number of glucose receptor sites decreases so that ever less glucose can be received and used by the cells. Without sufficient insulin to carry it into the cells where it can be used for energy, or without available receptor sites, glucose accumulates in the blood until some of the surplus is eliminated by the kidneys and passed off in the urine.

So typically when the ingestion of an over abundance of insulin inducing foods such as refined carbohydrates and convenience ‘junk’ food) occurs the pancreas can no longer produce enough insulin to restore optimal blood glucose levels. In this condition, the available insulin is ineffective, resulting in signs similar to complete insulin deficiency, hyperglycemia, (high blood sugar levels), glycosuria, (extra glucose spills over in urine), polydipsia, (great thirst leading to increased water intake), and weight loss.

How do you know you have Type II Diabetes?

Easy fatigue – because the body is not getting enough fuel.

Drowsiness, itching, blurred vision, excessive weight, tingling or numbness in the extremities, skin infections and slow healing of cuts and scratches especially on the feet. Because of the accumulation of sugar in the blood, the kidneys are hard-pressed to get rid of it, so you will probably be excessively thirsty and passing more urine than usual. Another prominent suspicious sign is an unexplained loss of sexual desire. Always seek advise from your medical advisor who can arrange the appropriate tests.

What can I do about this condition?

If you are a diabetic you are probably well aware of the effects of high glycemic foods and the need to maintain blood sugar surges. So what follows are a number of additional key recommendations:

•Eat a high-complex-carbohydrate, low-fat, high fiber diet including plenty of raw fruits and vegetables as well as fresh fruit juices. This

Reduces the need for insulin and also lowers the level of fats in the blood. Fiber helps to reduce blood sugar surges. For snacks, eat oat or rice bran crackers with nut butter or cheese. Legumes, root vegetables, and whole grains are also good.

•Avoid saturated fats and simple sugars (except when necessary to balance an insulin reaction).

•Get your protein from vegetable sources, such as grains and legumes. Fish and low-fat dairy products are also acceptable sources of protein.

•Supplement your diet with spirulina. Spirulina helps to stabilize blood sugar levels. Other foods that help normalize blood sugar include berries, brewer’s yeast, dairy products (especially cheese), egg yolks, fish, garlic, kelp, sauerkraut, soybeans, and vegetables.

•Do not take fish oil capsules or supplements containing large amounts of para-aminobenzoic acid (PABA), and avoid salt and white flour products. Consumption of these products results in an elevation of blood sugar.

•Do not take supplements containing the amino acid cysteine. It has the ability to break down the bonds of the hormone insulin and interferes with absorption of insulin by the cells.

•Avoid taking large amounts of vitamin B3 (Niacin). However, small amounts (50 to 100 milligrams daily), taken as a supplement may be beneficial.

Circulatory effects of Diabetes – How does that effect me?

For reasons not completely understood, Type II diabetes if untreated, can lead to vascular diseases that among other things, causes blindness, atherosclerosis, heart attacks, kidney disease, and gangrene. What we do know for certain is that in excess, insulin has the power to make arteries less elastic. It also acts as a growth hormone on smooth muscle cells in the walls of the arteries, causing them to increase in thickness, become stiffer and less supple, while at the same time decreasing the volume within the arteries.

Let’s look closer at effects that cause erectile dysfunction……

The subject of impotence usually remains behind closed doors, however, some physicians speculate that as many as 70 percent of diabetic men are impotent. In obese men this is related to an over abundance of oestrogen (a female hormone), which in turn desensitises the skeletal muscles to the action of insulin. Diabetes can damage the nerves, (neuropathy), the erectile tissue and small blood vessels in the penis. Neuropathy may mean there is less sensation in the penis and less signalling to it from the brain, making it more difficult to get and maintain an erection. Damage to the blood vessels will also prevent erectile tissue from filling with blood. This is why diabetes and erectile dysfunction are closely linked.

What other factors play a part in erectile dysfunction?

It may be that medications taken for diabetes, high blood pressure or for other conditions can be the cause. Drinking too much and smoking can also cause the problem. Psychological and emotional factors such as anxiety depression and stress are also important.

What you can do to overcome erectile dysfunction

Talk to your doctor. Because erectile dysfunction is a common diabetes-related problem, your doctor won’t be surprised when you mention the topic. The doctor can check if there are significant problems with the arteries or the nervous system as occasionally these aspects might need special attention in their own right.

Control your blood sugar. If blood glucose levels are kept in the normal range, it will help reduce the chance of this problem occurring.

Avoid tobacco. If you smoke-STOP! Smoking can cause blood vessels to narrow, contributing to blockages that can lead to erectile dysfunction. Smoking can also decrease nitric oxide levels, (which controls the circulation of blood, and transmits messages between nerve cells. As a result deficiency includes the inability to achieve and sustain normal erections).

Avoid excessive alcohol. As a rule of thumb men should restrict their intake to no more than two alcoholic drinks a day. Drinking excessive amounts of alcohol can cause erectile dysfunction by damaging blood vessels.

Maintaining normal blood pressure is important. In addition to smoking, and drinking excess alcohol please note that diabetics who control their condition reduce the risk of hypertension. I have 12 more strategies:

Watch your weight: In over weight people, a 10 percent reduction in total body weight will sometimes normalize blood pressure.

Compute your Body Mass Index: Multiply your weight in pounds by 703; then divide by your height in inches; then again divide by your height in inches. Try to stay between 18.5 and 24.9.

Regular exercise minimizes the chance of developing impotence and can restore sexual function in some men. In general, if you haven’t been exercising, try to work up to 30 minutes, 4 to 6 times a week. Please bear in mind the importance of eating more carbohydrates before exercise. Exercise produces an insulin-like effect in the body. Your doctor may make a different recommendation based on your health. If you can’t carry on a conversation while you exercise, you may be overdoing it. So in this case it is best to alternate exercise days with rest days to prevent injuries.

Beware of salt: No more than 6grams per day, many processed foods are sodium rich including soy sauce and canned soups.

Choose Potassium rich foods instead: Beware of Sodas, which deplete potassium.

Eat plenty of high fiber foods which are found in many vegetables and whole grains.

Reject refined foods: Natural wholefoods are the best – Not processed. Avoid fried foods, saturated/hydrogenated fats and avoid hidden fats, especially trans fats which are a greater risk than even saturated fats- check the food labels.

Importance of vitamin C: Make sure you have adequate amounts of this vital vitamin as it has been shown that the less vitamin C in the blood, the higher the blood pressure in hypertensive patients. Suggested dose with bioflavonoids such as Bilberry 3,000-6,000 mg daily, in divided doses.

Vitamin E: Evidence suggests that vitamin E also magnifies vitamin C’s blood pressure-lowering effects. Start with 100 IU daily and add 100 IU daily each month, until you reach 400 IU daily.

As much as possible avoid stress: Always ensure some “Me Time”, a great way to achieve this is putting 5 drops each of lemon balm and lavender essential oils in warm bath water.

Be sure to get sufficient sleep: High blood pressure patients deprived of sleep experience significant increases in blood pressure, especially during the evening.

These triglycerides are stored as fat. Unlike your muscles, your body has an unlimited storage potential for fat. The more fat you store, the more difficult it is to lose weight.

All of these excess triglycerides may not be removed from your blood stream and dumped into your fat cells. They could remain in your blood vessels. When this happens, your blood can actually thicken because there is an excessive amount of triglycerides in your bloodstream. When this happens, you are now a good candidate for a heart attack or stroke because or potential clotting or blockage in your blood steam.

Fiber

So, how can I prevent, or slow down this process? One way is to eat something that will absorb some of the carbohydrates, and dampen the insulin surge caused by the glucose. Fiber is a good thing to eat before or during your meal. Fiber helps you in two main ways. The first is it gives you a feeling of fullness that may cause you eat less at your major meals, and snack more during the remainder of the day. The second is that it binds or disrupts the full impact of the glucose conversion in your body. Many studies, in the United States and overseas, have shown very positive effects of adding fiber to your diet on a regular basis - lower body weight and lower waist-to-hip ratios. It also has been noted in many of these study groups that markers for heart disease were reduced.

Are all fibers created equally? No. Fiber from oats and barley are called beta-glucans. Beta-glucans have been shown to be very effective in slowing down the absorption of carbohydrates. It is recommended that you take five grams with or before each meal. Another fiber for your consideration is glucomannan. It is a dietary fiber that is actually a water-soluble polysaccharide. Glucomannan is effective in reducing the insulin spike after eating high-glycemic foods. Fiber has been shown to be an effective method to lose weight by reducing the number of carbohydrates that are absorbed. That doesn't mean that you can take extra fiber and enjoy a couple extra pieces of cake. Remain sensible and focus on your long-term goal - weight loss.

Dietary Supplements

Besides fibers, are there any other options? There are things other than fiber that can interfere or inhibit the breakdown of simple and complex carbohydrates. Oral anti-diabetic drugs called alpha-glucosidase inhibitors are used to treat type-2 diabetes. The alpha-glucosidase inhibitors prevent the digestion of carbohydrates and assist in weight loss.

White kidney bean extract is an alpha-amylase inhibitor. It blocks carbohydrate absorption by blocking the enzyme, alpha amylase which is used to break down carbohydrates in your digestive tract. The undigested carbohydrates are allowed to process through your body without absorption. It is a dietary supplement and 50 mg should be taken before each meal. You may also find dietary supplements containing both alpha-glucosidase and alpha-amylase inhibitors.

Responsibility

As always, you are responsible for your own health and weight loss is a good goal for a healthy future. Just because you might choose to take carbohydrate blockers or inhibitors, don't eat all the carbohydrates you want. Eat reasonably, take proper dietary supplements and exercise in moderation to achieve consistent and sustained weight loss. Blocking carbohydrate absorption might be an answer for some who have had difficulty in losing weight.

Seems at least once a week one of my patients asks me about the safety and effectiveness of some new diet formula they saw advertised on TV or the internet. The answer is a mixed bag of yes, no and maybe, depending on the ingredients in the formula and how they're being used. If you've got a few pounds to lose, it's only natural that you might be intrigued and tempted to try one of these formulas. So, allow me to share with you some information about a few common weight loss formulas out there, my recommendations, and let you to decide for yourself if you would like to try any.

Prescription Formulas

There are many prescription weight loss pills available, some control hunger, others rev up your metabolism, and still others play around with your insulin level to encourage weight loss. I feel that prescription weight loss formulas have health risks associated with using them. I would say, just leave the prescription formulas alone.

Over-The Counter Formulas

There are dozens of weight loss formulas out there. Most of them are herbal based. Here are some of the most common ingredients in these weight loss formulas, their function, and their health risks:

Metabolism boosters/Fat Burners:

Green tea extract - helps you burn fat by ramping up your metabolism. A downside is that green tea extract contains a lot of caffeine and could cause some negative effects such as diarrhea, dehydration, heart palpitations, and nausea.

White willow bark - an herbal derivative that aids in raising metabolism levels. It is also the herb that aspirin is derived from as it contains salicin, a natural pain killer. Often found in "stack" formulas, it doesn't do much on its own. If you are allergic, or have reactions to aspirin, you will want to avoid white willow bark as well. Like aspirin, it can decrease blood clotting and may interact poorly with prescription medications you may be taking.

Bitter orange - used in TCM (traditional Chinese medicine) for centuries for many conditions including weight loss. It contains the chemical synephrine which is similar to ephedrine in action, i.e., it revs up your metabolism like drinking caffeine.

Caffeine - boosts metabolism and can help you sustain exercise so you burn more fat and calories. Too much can dehydrate you, cause heart palpitations, jitteriness and insomnia.

Ephedra - the ban on this supplement was lifted not long ago but it has too many problems associated with it to use safely.

CLA (conjugated linoleic acid) - clinical studies have proven this formula effective in burning fat and preserving muscle. A little over 3 grams of CLA a day (3,400 mg) is needed to produce these beneficial effects. Possible side effects may be loose stools and nausea in some people but can be prevented if taken with food.

Garcinia cambogia - contains HCA, hydroxycitric acid, which inhibits the body's ability to store fat. No real known side effects, unless taken in very high does, but may have a laxative effect or cause stomach upset if too much is used at once.

Appetite controllers:

Hoodia - discovered in Africa, it was used by Bushmen to stop hunger pangs on long journeys. The real thing used in Africa may be a lot stronger than the manufactured type sold as a weight loss aid and depends on the integrity of the brand you buy and how they process the Hoodia. Some can be very expensive.

Irvingia gabonensis - from the West African dika nut, initial studies showed weight loss in the group studied. However, may have been due to decreased food intake, and may take as long as 90 days to kick in.

Carbohydrate/Sugar/Fat Blockers:

Phase 2 (white kidney bean extract) blocks carbohydrates from starches, i.e., pasta, potatoes, bread, corn from being absorbed.

Alli - (Orlistat/Xenical). You may have seen TV ads. Weight loss occurs as it blocks a percentage of the food you eat from being stored as fat and moves it quickly out of the intestines. In 2009 FDA received reports of liver injury, and investigations are underway.

5-HTP - increases "feel good" serotonin levels in the brain so you can stay on a carb-restricted diet longer to help lose weight. Also helps you sleep. As an adjunct to a healthy, lower carb diet, it can have some value in limited amounts.

Insulin Stabilizers:

Cinnamon - recent clinical studies were very promising in cinnamon by decreasing blood sugar levels and stabilizing insulin levels. Helps decrease belly fat by decreasing insulin. There is conflicting information, however, that common cinnamon, made from cassia bark, may be toxic to your liver in doses higher than 1 gram as it contains Coumarin. Ceylon or Sri Lankan cinnamon does not contain Coumarin.

Best to stay in the 1 gram (or two 500 mg capsule) limit. Diabetics should use with a doctor's supervision.

Chromium - a mineral, helps utilize insulin better, decrease belly fat deposits.

Gymnema sylvestre - an herb used in India for decades as a weight loss aid to lower blood sugar levels in diabetics, it is usually one of the ingredients in weight loss formulas along with chromium and cambogia gordonii. The American National Institute of Health deemed it helpful in controlling blood sugar levels in diabetics. However, diabetics should use under a doctor's supervision.

Cleansers:

Herbal slimming teas, or "Dieter's Tea" - contain rhubarb and senna which are powerful laxatives. These can be used sparingly/diluted as a pre-diet colon cleanse, but do not use these types of teas regularly. They can cause diarrhea and deplete your electrolytes which make your heart fire incorrectly with palpitations, etc.

Bowel cleansers - contain a lot of herbal ingredients like rhubarb, senna, cascara sagrada, and others. These work to clean the bowel of build up waste and can result in a few pounds of weight loss on the scale, but this is mostly water and waste loss and not fat. Useful as a pre-diet colon cleanse but not as a real fat loss solution.

Acai berry - another detoxing agent, ads for it are all over the internet touting huge amounts of belly fat loss and almost always by people who are selling it. The benefit of acai berry is in its vitamin, mineral, antioxidant content. The downside; it's not a magic bullet for weight loss.

Hormone/Sensory Formulas:

HCG (human chorionic gonadotropin) - HCG is a hormone produced during pregnancy present in urine. It is touted to ramp up metabolism so that you burn a lot of fat quickly. The downside to this routine is that you have to go on a very low calorie diet to accompany it, which may be impossible to tolerate, and is not cheap. Best used under a doctor's supervision.

Sensa - works on your sense of smell to decrease the amount of food you eat. If you've ever had a bad cold and couldn't smell you know how much your taste and desire to eat was also affected. Basically same principle. If you eat less, you lose weight.

My Recommendation

Weight loss is not all that complicated. It's basically a matter of eating less, but nutritionally dense, low sugar, amounts of food, and moving more, i.e., getting more exercise. I feel the best, natural supplements to aid weight loss are vitamins and minerals that support healthy fat and sugar metabolism like the array of B vitamins, L-carnitine, Vitamins C, E, D, and Omega-3's.
